SUMMER SPIFF CLAIM FORM
Graphtec America Inc. will pay the dealer’s individual salesperson for each eligible Graphtec model sold during the promotional period from May 25 to June 30, 2026.
All submissions must be received by July 15, 2026.

Copy of end-user invoice must be accompanied with every claimed item.
IMPORTANT: If your submission contains inaccurate information, your claim will not be processed. -
SIGNED W9 must be submitted with first claim form. Recipient is responsible for all applicable Federal and State Taxes. (Please ensure that your W9 has the current address.)
